Raffle – Official Rules

1. Eligibility

• Participation is open to all currently employed home healthcare staff within the organization.

• Participants must be in good standing at the time of entry.

2. Raffle Period

• The raffle will run for a specified time during the event

  • Nurses Week

  • Home Health & Hospice Month

• Entries must be submitted within this timeframe to be eligible.

3. How to Enter

• To receive one (1) raffle entry, employees must visit the office in person during the raffle period.

• Entries will only be granted upon physical check-in at the designated office location.

• Limit of one entry per employee unless otherwise specified.

4. Entry Verification

• Participants must sign in with their name and department upon arrival.

• Entries are non-transferable and must be submitted by the eligible employee only.

5. Prize(s)

• Prize details will be announced prior to or during the event.

• Prizes are non-transferable and cannot be exchanged for cash or other alternatives.

6. Winner Selection & Notification

• Winner(s) will be selected at random from all eligible entries.

• Drawing will take place on [date].

• Winner(s) will be notified via [email & phone].

7. Claiming Prize

• Winner(s) must claim their prize within [5] days of notification.

• If a winner does not respond within the allotted time, an alternate winner may be selected.

8. General Conditions

• By entering, participants agree to abide by these rules.

• The company reserves the right to modify or cancel the raffle at any time.

• The company is not responsible for lost, late, or incomplete entries.

9. Compliance

• This raffle is intended for internal employee engagement only and is not open to the public.

• All applicable company policies and local/state regulations apply.
