The Consultative Nurses are responsible for the following:
The overall coordination and monitoring of the health care needs for waiver participants.

Coordinate with hospitals and medical providers for care as needed, including admission and discharges.
Monthly in-home audits of overall care, medications, and schedules.
Our Consultative Nurses are always on-call for their clients. If a client’s nurse is unavailable, then Interim has escalation procedures in place for 24 hour on-call.
Interim HealthCare Consultative Nursing Services has 15 years’ experience with the IDD population and is the largest provider of Consultative Nursing Services in Delaware with over 350 clients managed by 10 full-time nurses devoted solely to their clients.
